MEDICATIONS AT SCHOOL
Students will not be allowed to carry prescription or over-the-counter (OTC) medication on their person, or store it in lockers, book bags, purses or gym bags. (The only exceptions are asthma medication and Epi-Pens.). This includes both short-term medications like prescription antibiotics and long-term medications like Ritalin.
Parent(s) may sign a form giving permission for their child to receive certain medications, which are provided by the parent and administered and stored by the nurse, if a dose is required during school. (For example - Tylenol, Ibuprofen, Benadryl, etc.) Your doctor must also complete a medication form for school for long-term administration of prescription medications like Ritalin. You must pick up ALL medications (including Inhalers and nebulizers and equipment) by the last day of school or they will be destroyed. THE NURSE CANNOT ADMINISTER ANY MEDICATION WITHOUT THE REQUIRED COMPLETED PERMISSION FORMS.
Students are allowed by law to carry and use asthma medications at school but a parent note will be required. ANY student who has been diagnosed with asthma must have an Asthma Action Plan on file with the nurse. This is also very important for athletes, who may require asthma medications during sports activities. The IHSA has rules about students using asthma inhalers during sports activities. The front portion of the sports physical must be completed & signed by both parent and physician to meet these requirements.
Medication sent to school that does not meet the rules for administration by the nurse must be picked up by parent(s) – and will NOT be sent home with students. (for example, antibiotics not in the original RX container, OTC meds not accompanied by parent note, pills in ziploc bags, carried in pockets and lunch boxes, etc). Cough/cold medications are not indicated for children 4 yrs of age or less and will not be given at school unless accompanied by a parent and a physician note.

Physical/Dental/Eye Exam Requirements
Early Childhood · School Physical - with completed immunizations & health history, on file at school within two weeks after child begins Early Childhood classes. Kindergarten · School Physical - with completed immunizations & health history, on file at school by October 15th of current school year. · Dental Exam – on file with school by May 15th of current school year, dental exam must be less than 18 months old as of May 15th of the current school year. · Eye Exam – Exam must be less than 1 year old on first day of school, on file at school by October 15th of current school year. 2nd Grade · Dental Exam - on file with school by May 15th of school year, dental exam must be less than 18 months old as of May 15th of the current school year. 6th Grade · School Physical - with completed immunizations & health history, on file at school by October 15th of the current school year. · Dental Exam – on file with school by May 15th of school year, dental exam must be less than 18 months old as of May 15th of the current school year. 9th Grade · School Physical - with completed immunizations & health history, on file by October 15th of the current school year. · Dental Exam - strongly recommended but not required. ***NOTE Eye exam is required for any student, any grade, who is enrolling in school from out of state, or enrolling in school for the first time. (ie after being home-schooled) |
